Since January 2006, the Transition Program managed by the HCMC Provincial AIDS Committee (PAC) and supported by USAID through PEPFAR has been established. FHI supported the development of a case management network to support the successful integration of returnees from drug rehabilitation centers to the community. Twenty eight case managers (CM) and drug addiction counsellors were recruited and equipped with skills and knowledge in drug dependence counselling, relapse prevention, HIV care and treatment; psychosocial support and involvement of families in the prevention of drug relapse.
